Our membership plans are different from insurance plans. We know not eveyone has dental insurance. We want to provide our patients with the best care possible while also making it affordable. Our In-House Membership Plans have one low annual (or monthly) fee and helps you save hundreds on a variety of dental services with no waiting periods, deductibles, or maximums. Membership runs for 12 months from the date you join.

​​​LIMITATIONS AND EXCLUSIONS:

Program Guidelines:

•This is not an insurance plan. Plan covers only services completed at Family Dentistry at City Park. Does not cover services completed at other dental offices.
•Payment must be made the day services are rendered to receive the plan discount.
•Cannot be used in conjunction with any other dental plan, discount plan or insurance plan.
•Plan is non-refundable and non-transferable. No refund will be issued if the plan is not used. The plan may not be cancelled within the 12 month membership period.
•If paid monthly, payment must be made by credit or debit card and authorized for 12 consecutive automatic monthly payments.

Plan cannot be used for:

•Cosmetic services (teeth whitening, veneers, cosmetic crowns, Smile Design, etc)
•Orthodontic services
•Procedures completed by specialists at other offices
•Should there be dental treatment needed following any type of injury where a lawsuit and, therefore, outside medical care, disability, workman’s compensation, accident insurance, automobile insurance, etc. are involved, this discount plan cannot be used.

Our membership plan is a direct payment arrangement for services, products and care provided.  It is not (i) insurance, (ii) a qualified health plan under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to discounted services.​​
